Supporting Positive Endings & Sustainable Change
Have you ever wondered...
- Why can ending support sometimes feel difficult for both practitioners and the people they support?
- How do I know when someone is ready to move on from social prescribing support?
- How can I promote independence while ensuring people feel empowered and connected?
- What does a safe, strengths-based and person-centred ending look like?
Ending support well can be just as important as the support itself. When approached positively and purposefully, endings can help people recognise their achievements, build confidence, strengthen community connections and continue their wellbeing journey independently.
This reflective workshop explores how positive endings can celebrate progress, promote autonomy and support people to build on the strengths, skills and networks they have developed through social prescribing.
By attending, you will:
- Explore why endings can be challenging for both practitioners and the people they support
- Reflect on boundaries, expectations and the emotional aspects of ending support
- Develop practical approaches to promoting independence and self-management
- Explore ways to recognise achievements and celebrate progress
- Increase confidence in planning and facilitating positive, person-centred endings
- Share experiences and learning within a supportive peer environment
